TWELVE-WAY FOUNDATION
Phase 2 (30 Days to 6 Months)


Program participation means all required meetings are met.

1. Will use sign board when leaving property.
2. Maintain savings plan .
3. Consider education needs.
Two day passes may be taken twice a month.
4. Prepaid drug screen required and a legitimate reason is a must.
5. Bi-monthly evaluations.
